New Patients: Registration

Registration couldn't be simpler. Just bring your existing medical card to either surgery, sign it, and we will initiate the process of retrieving your old notes from your previous surgery.

New NHS regulations now demand that we see ONE photo id eg a passport/driving licence, and ONE proof of address eg Utilities bill. These can be photocopied at the surgery and returned to you.

If the card has gone missing on your travels, don't worry: just come along, and you will be given a simple form to fill out in its place.

All newly-registering adults will be offered a Registration Medical Check with the Practice Nurse, but will need to see the Doctor before repeat prescriptions are issued.
